In generally it can be stated that you have to meet the health requirements for immigration purposes of the country where you want to work as an au pair.

Each country has it's own health requirements you have to meet. A health certificate is often required when your country of citizenship is rated with a high incidence of tuberculosis. On the homepage of the World Health organization you can find the actually global tuberculosis report. Also the individual immigration website of the relative country you've "chosen" gives you detailed information about these health reqirements.
